64yr male. no heart condition nor hypertension nor diabetes. no severe illness that need emergency care.? - foot drop edema

no known kidney or liver. The patient is of African origin. Seal between the proximal phalanx are swollen and painful, and can not bend. Pain is worse in the morning, directed especially when for a long day b4. inablity hold eating utensils. Swelling of the hands and feet rather than edema. He can not make a fist, and sometimes more than the pain radiates from the hand to the shoulder. is a bilateral condition. What is possible? We visited several doctors, but I need a good diagnosis. What do you think is a specialist and do u recommend? I am also a health and can diagnose a heart attack when I see, so do not worry. This is not a case of heart failure. It is a poor man in constant pain, can not eat in public bcos its so embarrassing when the owner drops his hands. also buy more shoes and more because of swelling in the feet. welcome a seasoned professional who helped me. Thanks

Perhaps a rheumatologist may be able to make a diagnosis. What kinds of tests are done already?
